respond promptly to transporrtation request,
assist patient to board and or disembarked from the program vehicle
work collabratley with CFSF and DPH employees in an integrated team to meet shared Goals
provide safe and courteous transportation services to patients
assist patients in activities of daily living (ADL)
coordinate wither and assist on-site Medical Respite staff to facilitate patient care
maintain daily logs of client contact, referrals, and transport
maintain good communication skills and mutual support with other service providers, city agencies
clean and maintain assigned vehicle
adhere to established policies and procedures
attend training and meetings requested
other related duties as assigned
Pay: From $25.00 per hour
