Care Professional - Chronic Care Management
Location: Richardson, TX (In-Office)
Compensation: $62,400 plus benefits (medical, dental, 401k, etc)

Wellness Care is the "glue between visits", serving America's most vulnerable populations, improving their health outcomes to reduce major health events and hospital readmissions. We provide concierge care, patient advocacy and ongoing support between appointments.
We are hiring patient-centered, wellness obsessed Care Professionals to join our Richardson clinical team. If you are passionate about direct patient care and developing long term relationships with your patients as you facilitate their care plans, this job is for you!
You are the day to day face of Wellness Care directly managing the care roadmaps of up to 140 patients. This is an in-office role where you work integrated with your Care Team. Care Teams are a team of six patient loving professionals, led by a Care Manager (RN) and comprised of 5 Care Professionals. The care team is supervised by an MD for robust clinical oversight. You will operate in a team environment where you are supported with a team and resources to provide the highest care quality to your patients.
You serve as the primary ongoing contact for patients enrolled in our chronic and transitional care programs.
What You’ll Do
- Serve as the main point of care -contact- for up to 140 assigned patients
- Conduct structured outreach and follow-ups
- Support implementation and monitoring of individualized care plans
- Reinforce provider recommendations and promote care plan adherence
- Coordinate labs, referrals, and follow-up appointments
- Identify clinical or social barriers and escalate concerns appropriately
- Document all patient interactions accurately in EMR/CRM systems
- Escalate patient concerns to Care Manager
- Participate in escalations as instructed by Care Manager and Supervising Physician

What We’re Looking For
- 2+ years of direct patient care experience (clinic, hospital, home health, EMT, MA, health coach, etc.)
- Experience supporting chronic condition populations preferred
- Comfortable working with Medicare-aged patients
- Strong empathy, communication and interpersonal skills
- Detail-oriented with solid documentation habits
- Tech-savvy and comfortable in digital healthcare systems
- Bilingual (English/Spanish) a plus
