The Lakes Home Care is looking for a motivated, organized, and detail-oriented
Intake Coordinator to join our growing team!
Responsibilities:
- Coordinate new patient referrals and admissions
- Communicate with physicians, hospitals, and referral sources
- Verify patient information and required documentation
- Schedule and coordinate start of care
- Work closely with our clinical team to ensure a smooth intake process
- Maintain accurate records and provide excellent customer service
Qualifications:
- Strong communication and organizational skills
- Ability to multitask in a fast-paced environment
- Experience in home health or healthcare is preferred but not required
- Proficiency with computers and Microsoft Office
