Audubon Hospice is growing and we are seeking a compassionate and dedicated RN Hospice Case Manager to join our dynamic healthcare team. Responsibilities
- Conduct thorough patient assessments to determine eligibility for hospice services, including vital signs, physiological status, and medical history review.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams, focusing on symptom management and patient comfort.
- Coordinate discharge planning and post-acute care transitions to ensure seamless continuity of care across inpatient, outpatient, and home settings.
- Manage intake processes for new patients, including medical records review, diagnostic evaluation, and utilization management.
- Provide education to patients and families about disease progression, treatment options, and hospice services to support informed decision-making.
- Monitor patient progress through regular visits; adjust care plans as needed based on clinical findings and patient preferences.
- Maintain accurate documentation in medical records while adhering to HIPAA regulations; ensure compliance with all healthcare standards and protocols.
- Collaborate with physicians, social workers, therapists, and other healthcare professionals to optimize patient outcomes and facilitate effective communication.
Experience
- Demonstrated expertise in hospital medicine or post-acute care settings with a focus on patient service and assessment.
- Prior experience in hospice or palliative care is preferred but not required; relevant background in geriatrics, behavioral health, or pediatrics is advantageous.
- Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and discharge planning procedures.
- Familiarity with medical records management systems and outpatient clinic operations is beneficial.
- Experience working within managed care frameworks or utilization management processes enhances your ability to coordinate comprehensive patient care effectively.
- Excellent communication skills combined with a compassionate approach to patient service are essential for success in this role.
