Occupational Therapist (OT) – PRN
Location: Kendallville Manor – Kendallville, IN
Revital Rehab Services is seeking a compassionate PRN Occupational Therapist (OT) to join our team at Kendallville Manor, a skilled nursing and rehabilitation facility in Kendallville, IN. In this role, you will provide high-quality therapy services designed to improve residents’ independence, functional abilities, and overall quality of life.
Implement individualized treatment plans developed in collaboration with the rehabilitation team
Conduct comprehensive evaluations and establish measurable short- and long-term therapy goals
Deliver evidence-based occupational therapy interventions to support residents’ daily living skills and functional mobility
Educate residents, families, and caregivers on techniques and strategies that support continued progress and safety
Collaborate closely with interdisciplinary team members to coordinate comprehensive resident care
Maintain accurate and timely documentation in accordance with facility and regulatory guidelines
Active Occupational Therapist license (OTR/L) in the state of Indiana
Degree from an accredited Occupational Therapy program
Strong communication and interpersonal skills
Commitment to patient-centered care and quality outcomes
Experience in skilled nursing or rehabilitation settings preferred, but not required
