What You’ll Do (PT, PTA, OT, OTA, SLP):
- Provide skilled physical, occupational, or speech therapy services in accordance with applicable laws, physician orders, payer requirements, and authorized plans of care.
- Evaluations & Care Plans (PT / OT / SLP): Perform comprehensive home evaluations, establish personalized plans of care, set functional goals, and adjust treatment plans based on clinical judgment.
- Interventions & Treatment (PT / PTA / OT / OTA / SLP): Deliver targeted therapy interventions to support mobility, daily activities (ADLs), speech and language skills, cognitive-communication, swallowing safety, strength, balance, and functional independence.
- Assessments & Education: Assess home environments, communication strategies, and swallowing/dietary safety to recommend equipment, modifications, adaptive techniques, or caregiver education as clinically appropriate.
- Collaboration: Communicate effectively with referring agencies, physicians, patients, families, supervising therapists, and interdisciplinary team members.
- Documentation: Document patient progress accurately and efficiently using digital tools in compliance with payer, agency, and regulatory requirements.
What We’re Looking For:
- Current California license as a Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupational Therapist (OT), Occupational Therapy Assistant (OTA), or Speech-Language Pathologist (SLP)
- Reliable transportation and valid driver’s license
- Professional liability insurance
- Passion for providing exceptional patient care
- Professional, responsive, and committed to high-quality documentation
- Ability to provide services as an independent contractor
