We are looking for a part-time or full-time Physical Therapist to join our growing rehabilitation clinic. This position is based at our outpatient clinic in Deerfield, IL
Responsibilities: *
- Evaluate patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
- Establish appropriate, functional and objective goals based upon clinical examination.
- Provide exceptional patient care and service.
- The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
- Cultivate relationships with patients, doctors and team members.
- Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
- Maintain patient charts, clinical logs and checklists necessary for compliance.
- Maintain patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
- Communicate well in a professional setting.
- Perform additional duties as required
Education and/or Experience
- Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program
- Current State of Illinois Physical Therapy license
- CPR Certified
