As a PRN Hospice Field RN, you’ll deliver compassionate, one-on-one care to patients facing life-limiting illnesses—primarily in the place they call home. You’ll have the autonomy to make clinical decisions in the field while being fully supported by a collaborative interdisciplinary team.
This role is ideal for an experienced hospice RN who values flexibility, independence, and deeply meaningful work.

Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs

Determine hospice eligibility, admission, and recertification using established clinical criteria

Develop, implement, and adjust individualized plans of care to ensure comfort and quality of life

Provide education and emotional support to patients and families on disease progression, end-of-life care, and self-care techniques

Coordinate with the interdisciplinary group (IDG) including physicians, social workers, and chaplains

Assess caregiver capability and identify support needs throughout the care journey

Supervise and provide clinical direction to Hospice Aides and LVN/LPNs

Document timely and accurate clinical and progress notes

Communicate changes in patient condition and collaborate on care adjustments

Active Registered Nurse (RN) license

CPR certification
-
Valid driver’s license and auto insurance

Minimum 1 year of hospice experience preferred (2+ strongly preferred)

Experience with Homecare Homebase is a plus
