Day-To-Day:
-
Delivers individual and group wellness coaching and education.
-
Plans and facilitates health, wellness, and social engagement activities.

Connects residents with healthcare, behavioral health, and community resources.
- Coordinates referrals and follow up on resident needs and goals.
- Maintains accurate documentation and assist with program reporting.

Collaborates with Service Coordinators, Community Administrators, and community partners.
- Supports residents in accessing medical, nutritional, behavioral health, and social services.

Monitors resident progress and provide follow-up support to address barriers and promote successful outcomes.
- Documents resident interactions, activities, referrals, and outcomes in required systems.

Tracks participation and program metrics to meet organizational and grant requirements.

Assists with program reports, case reviews, and quality improvement activities.
- Works closely with Service Coordinators, Community Administrators, healthcare providers, and community partners.

Participates in required training, team meetings, and ongoing professional development.
Qualifications:
-
Bachelor's degree in social work, Public Health, Psychology, Human Services, or a related field preferred; at least 2 years of experience working with older adults or in community-based health or social services.

Experience facilitating groups or leading educational activities.

Ability to work independently while building collaborative relationships with residents, families, and community partners.

Valid Texas Drivers’ License, clear MVR, and ability to be insured.
