What You'll Be Doing
As a Physical Therapist, you'll provide highly individualized care to patients recovering from surgery, illness, injury, hospitalization, or chronic disease progression.
Responsibilities include:
- Performing evaluations and treatment visits in patient homes.
- Completing Start of Care assessments and admissions.
- Developing individualized treatment plans focused on mobility, balance, strength, and safety.
- Supervising and collaborating with Physical Therapist Assistants.
- Partnering with nurses, physicians, and other clinicians to optimize patient outcomes.
- Monitoring progress and adjusting plans of care accordingly.
- Completing documentation and OASIS assessments in accordance with regulatory requirements.
Qualifications
Current Minnesota Physical Therapist License (PT or DPT) Required
- Home health experience preferred but not required.
- OASIS experience preferred.
- Strong clinical judgment and decision-making skills.
- Excellent communication and relationship-building abilities.
- Ability to work independently and manage a territory.
- Valid driver's license and reliable transportation.
