Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide skilled physical therapy services in patients' homes.
- Improve mobility, strength, balance, and overall functional independence.
- Educate patients and caregivers on exercises, safety, and fall prevention.
- Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members.
- Complete accurate and timely documentation in accordance with Medicare and agency guidelines.
- Participate in care planning and care plan meetings as needed
Qualifications
- Current Florida Physical Therapist (PT) license.
- Graduate of an accredited Physical Therapy program.
- Valid Florida driver's license and reliable transportation.
- Excellent communication and interpersonal skills.
- Home Health experience preferred.
- OASIS documentation experience is a plus but not required.
