Job Overview
We are seeking compassionate and dedicated Certified Nursing/Home Health Aides to provide exceptional care to our clients in a variety of settings, including home health, assisted living, nursing homes, and hospice care. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and experience working with diverse populations, including individuals with developmental disabilities, dementia, and other chronic conditions. Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
- Monitor and observe patient conditions, reporting any changes or concerns promptly
- Administer medications and document medication administration accurately using EMR/EHR systems
- Perform vital signs checks and record medical data in compliance with HIPAA regulations
- Support patients with mobility using equipment such as Hoyer lifts and assist with transfers safely
- Provide specialized care for individuals with Alzheimer's disease, dementia, or memory care needs
- Assist with catheter care and manage other medical procedures as trained
- Prepare meals according to dietary guidelines and assist with feeding when necessary
- Perform light housekeeping tasks including laundry and cleaning to maintain a safe environment
- Document all patient interactions and care activities thoroughly in medical records
- Follow all safety protocols, including CPR certification and infection control procedures
Skills
- Proven experience in patient observation and caregiving within assisted living or long-term care environments
- Familiarity with EMR/EHR systems and medical documentation standards
- Strong knowledge of ADLs assistance, medication administration, and medical terminology
- Experience working with individuals with disabilities, developmental delays, or chronic illnesses such as Alzheimer’s or dementia
- Ability to perform CPR and catheter care confidently; hospice care experience is a plus
- Competence in using mobility aids like Hoyer lifts and taking vital signs accurately
- Compassionate caregiver with excellent communication skills and patience
- Knowledge of HIPAA regulations to ensure patient confidentiality at all times
- Ability to work independently or as part of a team in home & community care settings
- Flexibility to work live-in employment arrangements if required
