Schedule: Monday – Friday, 40 hours per week

About the Role

Key Responsibilities
- Evaluate, plan, and implement individualized treatment programs for patients in an outpatient setting.
- Utilize evidence-based practices to help patients regain strength, mobility, and independence.
- Collaborate with referring physicians and interdisciplinary teams to ensure optimal patient outcomes.
- Maintain accurate and timely documentation of patient progress and treatment plans.
- Provide education and support to patients and their families regarding home exercise programs and wellness.
Qualifications
- Education: Graduate of an accredited Physical Therapy program.
- Licensure: Current Physical Therapist license in the state of Kentucky (or eligibility to obtain).
- Experience: Minimum 1 year of outpatient physical therapy experience required.
- Certifications: BLS certification required.
