Position Summary
Responsible for all habilitative programs and programing for IWC Residential programs. Provides direct clinical supervision of all direct care staff.
Essential Duties & Responsibilities

Lead The Habilitation team for each consumer.

Plan and write consumer programs and annual Habilitation plans.

Submit Bi & annual service authorization requests

Be aware of recommendations after Utilization Review.

Ensure State and Federal regulations are followed at all times.

Serve as a community liaison with school systems and other community organizations and agencies.

Promote, coordinate and/or assist in planning new ideas and programs.

Plan write and implement plans of correction (POC) from an State survey.

Intake all new consumers and ensure required procedure is followed.

Review all Resident Incident Reports. Look for patterns.

Ensure continued communication among all IWC/IWRC facilities.

Communicate with parents, and serve as a mediator as needed.

Provide Paraprofessionals with Clinical Supervision per the APSM 30-2

Oversee the overall operations of Consumer residents.

Supervises and annually evaluates the home managers assigned to case load.

Carry out or assist in corrective actions.

Uphold and monitor all IWC/IWRC policies and procedures in the houses.

Other duties as assigned.
Education/Training
Must have at least a Bachelor’s Degree, with at least two years’ experience working directly with IDD population. Must meet State regulations for QP. Related supervisory experience required.
Necessary Special Qualifications
Contacts Inside/Outside Organization
IWC/IWRC staff, employees, parents/guardians, client families, School Systems and day program staff, Care Coordinators etc.
Other Qualifications or Requirements
Mental Effort: Ability to set priorities, follow through, develop systems, and maintain accountability for staff job performances. Ability to understand and follow complex verbal & written instructions, technical and computer language as well as legal terminology. Ability to function independently. Good problem solving ability.
Visual Effort: Well-lighted office environment requiring average amount of visual effort.
Physical Requirements: Office environment not usually requiring ongoing strenuous physical qualifications. May require occasional lifting of 40 pounds. Must be free from communicable diseases. Annual physical and TB test up to date.
Safety: IWC’s philosophy is to provide the safest work environment possible while meeting requirements from OSHA, Life Safety, Health Department, Fire Marshall, Survey team and other regulatory agencies.
Working Conditions: Normal office environment which is not exposed to extremes in temperature, humidity, dust or pollutants. Maybe required to report during evenings, weekends, etc. as necessary.
Scope of Position: Number of employees supervised directly – 2 or more.
