Otterbein has an employment opportunity for a Care Coordination Specialist. This role with collaborate with the Admissions Specialists, HealthCare Navigators (marketing) and community staff to ensure residents and families receive outstanding customer service upon arrival and throughout their stay.
The Care Coordination Specialist will:
- Lead tours for prospective residents and families
- Develop and maintain a list of potential new long-stay referrals
- Oversee all case management and discharge planning of residents during their skilled stay
- Manages appeals processes with insurance companies
- Supports business office functions
