We are seeking dedicated Registered Nurses (RNs) and Licensed Nurses ( LPN) who are passionate about delivering high-quality patient care while enjoying flexibility and independence.
As an RN /LPN Field Nurse, you will provide skilled nursing care, develop individualized care plans, educate patients and families, and collaborate with the healthcare team to help patients remain safe, healthy, and independent in their homes

Responsibilities
- Complete comprehensive patient assessments and develop individualized plans of care in collaboration with the physician.
- Provide skilled nursing care, including medication administration, catheter care, tracheostomy (trach) care, disease management, and symptom management.
- Educate patients and caregivers on medications, disease processes, safety, and self-care.
- Coordinate care with physicians, therapists, pharmacists, and other healthcare providers.
- Complete accurate electronic documentation within agency guidelines.
- Follow all infection control policies and state, federal, and agency regulations.
- Travel to patient homes within your assigned territory while managing your daily schedule.
Qualifications
- Active, unrestricted New Jersey Registered Nurse (RN) or Licensed Practical Nurse ( LPN) license.
- Current BLS/CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Excellent communication and organizational skills.
- Ability to work independently in the field.
- Ability to lift, push, and pull up to 50 pounds.
- Ability to pass a background check, TB/health screening.
