For over 30 years, St. Mary's Center Skilled Nursing Facility (40 bed SNF) has delivered high-quality medical care and essential social services to people living with HIV/AIDS in Harlem and New York City. Within the next five years, St. Mary’s Center is expanding our services beyond caring for People Living With AIDS (PLWA) to include primary care services for the underserved, vulnerable community at large. St. Mary’s Center is hiring for a Director of Resident Services responsible for the day to day operations of the Resident Services Department for the nursing home.
RESPONSIBILITIES:
- Supervise admission and discharge planning.
- Participate in the interdisciplinary team care planning process (CCP); serve as team leader and facilitate the care plan meetings as needed.
- Carry a reduced case management caseload; provide one-on-one counseling for a selected caseload.
- Provide support groups for family members, significant other(s), and care givers.
- Provide and maintain appropriate documents, records, charts, monthly reports, and statistical data in an organized, accurate, and timely manner.
- Participate in the NYSDOH annual survey; provide plan of correction documentation when necessary.
- Establish and revise policies and procedures for the department.
- Custodian of department budget.
- Educate residents and staff on Resident Bill of Rights and PattP.nt Abuse Law.
- Involvement in bed retention, room changes, and resident transfers.
- Member of Quality Assurance Committee and provide AQI studies; understanding of employee roles in the QAPI program.
- Supervise mental health manager; coordinate staffing coverage on weekends and evenings.
- Attend off-site meetings, trainings, and seminars as assigned.
- Facilitate AOD schedule.
- Custodian of resident psychosocial and medical information.
- Perform other duties as assigned by NH Administrator.
QUALIFICATIONS:
- Masters Degree in Social Work from accredited college or university. LMSW preferred.
- Experience in working in a healthcare setting especially with people living with AIDS
- Bilingual (Spanish) preferred
Pay: $66,000.00 - $70,000.00 per year
