90+ year old man who lives alone in an apartment within the community. His medical diagnoses include Shortness of Breath, Hypertension, Coronary Heart Disease. The patient will need constant supervision from the care giver to prevent fall, Laundry, running errands for the patients, light breakfast, making sure he is well groom and in a clean environment.
