Roles and Responsibilities: NoVa Permanent Supportive Housing and NoVA Intensive Clinical Resource Coordinator
NoVA Permanent Supportive Housing – Responsibilities
Complete all documentation pertaining to each client’s individualized annual service plan, transferring levels of care, and discharge.
Such as: Annual Individualized Treatment Plan, Quarterly Reviews, Annual Reassessments, Transfer Assessments, Transfer Summaries, and Discontinue Treatment Plans.
Create treatment plans tailored to the areas of improvement identified by the client and staff, utilizing the client's comprehensive DLA-20 score and highlighting short and long-range goals.
Provide thorough and detailed reviews on a quarterly basis, utilizing information gathered from interviewing the client, staff reports, medication administration records, and the client's comprehensive DLA-20 score for the quarter.
Complete Annual Assessments in accordance with each client’s admission date anniversary.
Responsible for the completion of Transfer Assessments and Summaries for clients transferring from different levels of care throughout Gateway to NoVA Permanent Supportive Housing. In addition to discontinuing treatment plans when clients are discharged.
All documentation is to be completed in a timely manner in accordance with each individual's unique admission date and the start/end date of their treatment plan.
Assist the PSH Coordinator in engaging in skills-building and providing assistance to clients.
Assisting clients in: problem-solving, obtaining medications, transportation to certain injection appointments, and additional skills-building as needed by the client (help with cleaning their apartments, navigating school and employment, tailoring resumes, and learning new recipes, etc.)
On call during shift to answer all communication efforts made by clients.
Complete monthly check-ins with PSH Rental Only clients via phone calls.
Assist PSH Coordinator in maintaining clients' individualized green binders to remain in compliance with licensing and audit requirements.
Maintain the accuracy of the Clinical Documentation Tracker.
NoVA Intensive – Responsibilities
Complete all documentation pertaining to each client’s individualized annual service plan, transferring levels of care, admission/discharge, and legal complexities.
Such as: Preliminary Plan, Preliminary Review, Full Assessment, Initial & Annual Individualized Treatment Plan, Quarterly Reviews, Annual Reassessments, Transfer Assessments, Transfer Summaries, and Discontinue Treatment Plans.
Create treatment plans tailored to the areas of improvement identified by the client and staff, utilizing the client's comprehensive DLA-20 score and highlighting short and long-range goals. (Including legal intricacies in compliance with individualized release plans)
Provide thorough and detailed reviews on a quarterly basis, utilizing information gathered from interviewing the client, staff reports, medication administration records, and the client's comprehensive DLA-20 score for the quarter.
Coordinate directly with Team Leads to schedule meetings with clients, obtain quarterly DLA-20 scores, receive staff reports, and communicate any crucial information.
Coordinate with Team Leads and NoVA Intensive Director to prepare and execute new admissions.
Responsible for the completion of all new admissions to Gateway NoVA Intensive, admission-related documents, and orientation procedures for each newly admitted client.
Such as: admission packet documents, Preliminary Plan, Preliminary Review, Initial Plan, and Full Assessment.
Responsible for creating and organizing green binders for newly admitted clients.
Complete Annual Assessments in accordance with each client’s admission date anniversary.
Responsible for the completion of Transfer Assessments and Summaries for clients transferring from different levels of care throughout Gateway to NoVA Intensive. In addition to discontinuing treatment plans when clients have been discharged or have transitioned levels of care.
All documentation is to be completed in a timely manner in accordance with each individual's unique admission date and the start/end date of their treatment plan.
Maintain the accuracy of the Clinical Documentation Tracker.
Pay: $27.08 - $32.61 per hour
