Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history reviews to develop tailored care plans.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and medical documentation standards.
- Provide specialized care for individuals with complex conditions such as dementia, Alzheimer's disease, or developmental disabilities, ensuring safety and comfort.
- Manage wound care, catheterization, tube feeding, ventilator support, and other advanced clinical procedures with precision and compassion.
- Collaborate with families, physicians, and interdisciplinary teams to coordinate discharge planning, case management, and ongoing health coaching.
- Maintain accurate electronic health records (EHR/EHR systems) while adhering to HIPAA confidentiality policies and long-term care regulations.
- Respond swiftly to urgent situations through triage skills and acute pain management techniques to stabilize patients effectively.
Skills
- Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal or pediatric ventilator care, and individuals with disabilities.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation procedures.
- Skilled in using EMR/EHR systems such as Cerner or Athenahealth for efficient documentation and patient record management.
- Expertise in infection control practices, sterile techniques, specimen collection/processing, and aseptic procedures.
- Ability to perform advanced clinical tasks such as IV insertion/insertion site care, venipuncture, suturing assistance, and airway management.
- Familiarity with emergency medicine protocols including Level I Trauma Center experience or ICU experience is highly desirable.
- Excellent communication skills for patient assessment discussions and family education while maintaining clinical confidentiality policies.
