Newfane Rehabilitation & Health Care Center, a warm, community-centered skilled nursing and rehabilitation facility in the heart of Newfane, NY, is seeking an Occupational Therapist (OT) to join our therapy team. Our facility provides both short-term rehabilitation and long-term care in a close-knit, small-town setting where staff and residents know each other by name. The Occupational Therapist (OT) plays a central role in helping residents regain independence and return home. Prior skilled nursing facility (SNF) experience is not required — we welcome any licensed Occupational Therapist (OT) ready to make a difference.

Evaluate residents and develop individualized occupational therapy treatment plans
Provide skilled occupational therapy to short-term rehab and long-term care residents
Document evaluations, progress, and outcomes accurately and timely
Collaborate with the interdisciplinary care team on resident goals
Educate residents and families to support safe discharge and independence

Salary: $92,000 – $100,000 per year
