About the Role
We are looking for a dedicated and self-directed Full-Time RN Case Manager(Hybrid) to join our hospice team in the Los Angeles area. If you value holistic patient care and are looking for a stable, routine Monday-to-Friday schedule with minimal travel, this is the perfect opportunity for you.
Job Description & Key Responsibilities
- Case Management: Manage a manageable caseload of approximately 15 hospice patients.
- Patient Visits: Conduct routine visits (typically once every 2 weeks per patient).
- Office Work: Focus on case management, coordination, and documentation from the office on non-visit days.
- Admissions & Triage: Handle occasional admissions or on-call triage as needed.
- Minimal Travel: Most patients are clustered in nearby areas, significantly reducing travel time.
Qualifications & Skills
- License: Active Registered Nurse (RN) license in California (Required).
- Language: Bilingual in Korean and English (Highly Preferred) to effectively communicate with our Korean-speaking patients and families.
- Attributes: Self-motivated, organized, and passionate about delivering compassionate, holistic end-of-life care.
- Hospice experience is a plus, but NOT required. We provide thorough training!
Schedule & Compensation
- Schedule: Monday – Friday, 9:00 AM – 5:00 PM
- Schedule consists of 3 days working from home and 2 days in the office/field
- Pay Rate: Competitive salary, negotiable based on your experience and qualifications.
