Primary Care Physician | $100K Sign-On Bonus | Outpatient | Avondale, AZ
Provenir Healthcare is seeking a Board Certified Internal Medicine or Family Medicine Physician to join a growing outpatient primary care practice in Avondale, Arizona.
This full-time opportunity offers physicians the chance to practice medicine in a value-based care environment focused on quality outcomes, preventive care, and long-term patient relationships. You will care for an adult Medicare Advantage population while being supported by a dedicated clinical team and resources designed to help you provide exceptional patient care.
Unlike traditional high-volume primary care settings, this practice is designed around manageable patient volumes, coordinated care, and giving physicians the time needed to truly connect with their patients.
Position Highlights
- $100,000 Sign-On Bonus with a 3-year agreement
- Full-time, employed position
- 100% outpatient primary care
- Monday-Friday schedule (8:00 AM – 5:00 PM)
- No evenings, weekends, or holidays
- Limited telephone-only call responsibilities
- Value-Based Care model focused on quality outcomes—not RVUs
- Smaller patient panels that support relationship-based care

What You'll Do
- Provide comprehensive primary care to adult Medicare Advantage patients
- Diagnose and manage acute and chronic medical conditions
- Focus on preventive medicine, wellness, and chronic disease management
- Build long-term patient relationships through continuity of care
- Collaborate with physicians, specialists, care coordinators, and support staff to improve patient outcomes
- Maintain accurate and timely clinical documentation while delivering compassionate, patient-centered care
Qualifications
- MD or DO required
- Board Certified in Internal Medicine or Family Medicine required
- Active, unrestricted Arizona medical license (or ability to obtain)
- Current DEA registration or ability to obtain
- Previous outpatient primary care experience preferred
- Experience caring for Medicare Advantage patients preferred
- Value-based care experience is a plus
