Position Summary: The Homeless Shelter Case Manager is responsible for coordinating services for clients of the Haven Center and the provision of accurate and complete record keeping while working on Case Management on and off-site. Responsibilities include creating case plans for clients that initiate participant referrals for jobs and housing and provide feedback for progress and motivation towards successfully acquiring income and permanent or transitional housing opportunities. Has knowledge of and assists with UCAN and EFSP funds for shelter recipients as it relates to housing.
Essential Job Functions:
- Creates intensive, individualized case management plans by coordinating client goals with available supports and resources
- Provides oversight and support for Haven Center participants while on premises, in off-site shelter or during Naomi’s House programming
- Assesses and documents individuals for program eligibility.
- Provides information and referrals to community resources, including mental health, AODA, medical, educational, financial services, based on assessment.
- Documents case files on a regular basis/tracks interaction with participants
- Maintains case records in a timely manner.
- Ensures case records are available for generating reports, audits, grant writing, supervision.
- Uses Case Management tool to track statistical and geographical information which is not entered into HMIS for monthly reporting.
- Checks and maintains current roster of participants
- Encourages and supports participants in locating employment and permanent housing
- Collaborates with community partners to provide quality programming for participants
- Follows up to ensure that clients are connected to and utilize referral services.
- Submits and reviews reports with supervisor.
- Consults with supervisor regarding problems or concerns.
- Acts as Caseworker for UCAN (United Community Assistance Network) Program and EFSP for shelter residents
- Other duties as assigned
Additional Job Functions:
- Coordinates with other Neighborhood Alliance programs, such as child care or Help Me Grow, to support participants
Necessary Education and Training:
- Licensed Social Worker preferred. Associates degree or higher in Social Work or Human Services is required.
- Must demonstrate good writing and verbal skills, computer skills a plus.
- Experience working with low income individuals and families preferred.
- Data Entry, Microsoft Excel and Word
- Ability to remain calm and think clearly in a crisis is essential
Pay: $16.00 - $18.00 per hour
