A large Licensed Home Care Services Agency (LHCSA) is seeking a Registered Nurse (RN) to serve as a Nurse Educator supporting home care patients and staff across Brooklyn and Queens. This role includes a combination of classroom instruction, home visits, clinical supervision, and care coordination.
Position Overview
The Nurse Educator will be responsible for delivering high-quality patient care in the home setting while also training, supervising, and supporting Home Health Aides (HHAs), LPNs, and other clinical staff. The role requires strong clinical judgment, teaching ability, and field-based home care experience.
Key Responsibilities
- Assess patient and family situations for nursing and health-related needs in the home setting
- Provide skilled nursing care to patients requiring intermittent or part-time professional nursing services
- Develop, implement, and coordinate individualized plans of care in collaboration with the interdisciplinary team
- Supervise, instruct, and evaluate care provided by HHAs, LPNs, patients, and caregivers
- Conduct home visits to monitor patient status and ensure quality of care delivery
- Report significant changes in patient condition to physicians and adjust care plans as needed
- Perform preventive, restorative, and rehabilitative nursing interventions
- Educate patients and families on disease management, self-care techniques, and prevention strategies
- Evaluate patient outcomes and effectiveness of care plans
- Coordinate referrals and collaborate with community agencies and healthcare professionals
- Interpret agency policies and regulatory requirements for patients and families
- Maintain accurate, timely, and compliant clinical documentation in accordance with agency and regulatory standards
- Ensure confidentiality and proper management of patient records
- Support discharge planning and continuity of care
- Participate in agency quality improvement, education programs, and training initiatives
- Assist with orientation and mentoring of new clinical staff and student nurses
- Maintain required productivity standards and contribute to agency goals
- Engage in ongoing professional development and continuing education
- Perform other duties as assigned
Qualifications
- Current RN license and registration in the State of New York (required)
- Minimum 1–2 years of experience in Medical/Surgical nursing or home care
- At least 1+ year of home care experience required
- Bilingual in Cantonese and Mandarin (required)
- Strong ability to assess and respond to patient and family needs in a home setting
- Valid driver’s license and reliable vehicle for field visits
- Professional liability insurance (required)
- Proficient with basic computer systems and electronic documentation
- Strong communication, teaching, and organizational skills

Pay: $114,000.00 - $115,000.00 per year
