Rural Health Program Coordinator

This is a highly independent role. You will hold yourself accountable to deadlines and grant deliverables, and build relationships across the community — often without someone looking over your shoulder. What You'll Do
Program Coordination
- Plan and execute rural health initiatives aligned with grant objectives, including the Rural Health Network Development program
- Coordinate workshops, trainings, and outreach activities focused on health education and awareness
- Ensure all program activities meet state and federal guidelines and grant compliance requirements
Community Engagement & Outreach
- Build and maintain partnerships with healthcare providers, schools, community organizations, and local agencies
- Conduct targeted outreach to underserved populations to increase access to services
- Represent the organization at community meetings, events, and coalitions
Health Education & Awareness
Organize and facilitate workshops on topics including:
- Preventive care and chronic disease management
- Mental health awareness
- Access to healthcare resources
- Develop culturally appropriate educational materials tailored to the communities you serve
Data Collection & Reporting
- Track program metrics, participation, and outcomes with precision
- Maintain accurate, organized records for reporting requirements
- Prepare reports for funders and state agencies; meet all grant deliverables and deadlines without fail
Network Development
- Strengthen and expand partnerships across the rural health network
- Support coordination between healthcare systems and community-based organizations
- Proactively identify gaps in services and bring solutions to the table
Pay: $20.00 - $25.00 per hour
