Extended Care Clinical is seeking a dynamic, relationship-driven Business Development Liaison to join our team and support our skilled nursing facilities throughout the Greater Chicago area, with a focus on the southern Chicago suburbs.

The Business Development Liaison serves as a key connection between our skilled nursing facilities and the hospitals, healthcare providers, case managers, discharge planners, physicians, patients, and families we serve.
This position is responsible for developing and maintaining referral relationships, coordinating the referral and admissions process, educating referral partners about our services, and ensuring that patients receive a smooth and well-coordinated transition of care.
Receive and respond promptly to referral inquiries from hospitals, healthcare providers, skilled nursing facilities, physicians, patients, families, and other referral sources.
Conduct on-site clinical assessments and provide education and in-service training to referring providers and facility staff.
Assist referral sources in identifying appropriate patients for admission and educate them regarding admission criteria, services, and capabilities.
Communicate transfer, admission, clinical, financial, and insurance information to appropriate facility team members to ensure the facility is prepared for each admission.
Meet with patients and families to explain the transfer and admission process, anticipated treatment, expected outcomes, services, and financial considerations.
Obtain necessary medical, demographic, insurance, and financial information to support completion of the admissions process.
Serve as a primary resource and point of contact for referral sources regarding facility services, capabilities, admissions, and patient needs.
Establish and maintain strong, professional relationships with hospitals, physicians, discharge planners, case managers, social workers, and other healthcare partners.
Conduct regular visits to assigned hospitals, physician offices, healthcare facilities, and other referral sources throughout the Greater Chicago area.
Identify opportunities to increase awareness of our services and strengthen referral partnerships.
Provide ongoing education to referral partners regarding clinical services, admissions criteria, care capabilities, and patient outcomes.
Work closely with facility leadership and clinical teams to ensure timely communication, responsiveness, and continuity of care.
Monitor referral and admission activity and identify opportunities for growth and improved customer service.
Represent Extended Care Clinical professionally and positively within the healthcare community.
Participate in meetings, committees, community events, and other organizational activities as required.
Participate in continuing education and in-service opportunities to maintain and enhance professional knowledge and skills.
Perform on-call professional duties from time to time, as needed.
Maintain confidentiality and comply with all applicable healthcare regulations, organizational policies, and professional standards.
Bachelor’s degree in Nursing, Healthcare Administration, Business Administration, Marketing, or another healthcare-related field preferred.
Minimum of two years of experience in a healthcare setting; experience in skilled nursing, post-acute care, hospital discharge planning, case management, or healthcare business development strongly preferred.
Clinical background or healthcare experience with the ability to understand and communicate patient care needs.
Knowledge of skilled nursing, post-acute care, admissions processes, and third-party reimbursement preferred.
Demonstrated ability to establish and maintain effective relationships with healthcare clinicians, providers, and referral partners.
Strong understanding of patient-centered care and the importance of a positive patient and family experience.
Excellent verbal, written, interpersonal, and presentation skills.
Highly organized with strong attention to detail and follow-through.
Self-directed, motivated, and able to work independently with minimal supervision.
Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
Professional demeanor and ability to represent the organization positively in the community.
Valid driver’s license and reliable transportation required for travel throughout the Greater Chicago area.
