Position: Rehabilitation Specialist – Full Time
Location: Springfield, Ma
Rate of Pay: $26.00 per hour
Hours: 40 hours per week
Schedule: Monday – Friday 8:00am – 4:00pm with flexibility
Summary

The Rehabilitation Specialist is responsible for coordinating, implementing, and monitoring individualized services that support members in achieving their personal goals and objectives in accordance with Individual Support Plan regulations. This position gathers and analyzes information, documents member progress, presents goals and outcomes, and collaborates closely with case managers, families/guardians, and other service providers to ensure high-quality, person-centered services. Additional responsibilities include overseeing the screening and prioritization of referrals, implementing behavior support strategies, investigating incidents, maintaining accurate documentation, and performing other duties as assigned.

High School diploma or equivalent with a minimum of five (5) years of related experience; OR Associate's degree with three (3) years of related experience; OR Bachelor's degree with one (1) year of related experience required.

Previous experience supporting individuals with intellectual and/or developmental disabilities or other vulnerable populations is required.

Knowledge of and respect for individuals from diverse backgrounds.

Previous service coordination experience preferred.

Previous job coaching and/or supported employment experience preferred.

Experience using Credible electronic health record software preferred.
-
Minimum of two (2) years of previous case management experience is required.

Demonstrated case management experience that includes assessing an individual's strengths, limitations, support needs, and personal interests to develop person-centered service plans.

Experience developing, implementing, and maintaining ISP's is required.

Strong organizational, documentation, and time management skills with the ability to manage multiple priorities in a fast-paced environment.

Proficiency with Microsoft Word, Outlook, Excel, email, internet, and data entry.

Excellent written, verbal, interpersonal, and active listening skills.

Demonstrated commitment to providing exceptional customer service to participants, families, referral sources, and internal team members.

Valid driver's license, good driving record, reliable transportation, and willingness to transport participants in a personal and/or agency vehicle.
