Decide To Believe, LLC. is a growing mental health agency actively seeking qualified applicants for our Crisis and Mental Health Programs. Crisis services are designed to provide intensive short-term mental health care to non-hospitalized individuals experiencing an acute crisis of psychiatric nature. Services are provided on a short-term basis beginning at the time of assessment and require up to 4 hours of direct contact daily.
Crisis Counselors are required to assist in the formulation and implementation of clinical and diagnostic service plans related to the needs of the individual. The crisis counselor is also responsible for the day-to-day coordination of appropriate services and interventions that work together to stabilize the individual in the least restrictive manner/setting.
Essential Duties and Responsibilities:
· Collaborate with interdisciplinary teams to ensure coordinated care for clients
· Maintain accurate and up-to-date client records, ensuring compliance with HIPAA regulations
· Provide crisis stabilization services to adults, adolescents and children who are at imminent risk of hospitalization of out of home placement.
· In addition to direct client care, counselors are responsible for coordinating services according to clients’ treatment plans, facilitating therapeutic activities, engaging families in crisis intervention and stabilization services to provide complete wrap-around services, and facilitating admissions and discharges from the program.
· Maintain and ensure compliance with DBHDS and Medicaid standard
· Coordinate the development and timely submission of Individualized Service Plans and all other required program documentation
· Participate in weekly supervision meetings with LMHP
· Perform chart audits as needed to ensure compliance with all Medicaid and DBHDS Standards and Ensure all Serious Incident Reports are completed in a timely manner.
· Ensure all Human Rights reportable incidents are investigated and reported according to agency policies and all necessary steps are taken to protect clients
· Adhere to all agency policies/procedures and complete other tasks assigned by the Program Director deemed necessary for the successful operation of the program
Required Education, Credentialing, and Experience:
· Bachelor's degree in social work, psychology, or other related human services field required; Master's degree preferred.
· Valid license/certification as a Qualified Mental Health Professional
· Experience working with individuals with developmental disabilities or mental health disorders
· Two years of experience in the Mental Health field is preferred.
· One year of experience working in Crisis Services is preferred.
· Eligible applicants must possess a valid driver's license with a safe driving record.
Job Types: Full-time, Part-time, Contract
Pay: $28.00 - $35.00 per hour
Expected hours: 20 – 40 per week
