Primary Responsibilities
As a field-based LPN, you’ll work closely with patients and care teams to provide high-quality, compassionate care. Responsibilities include:
- Deliver care according to physician orders and established care plans
- Administer medications, wound care, and treatments as directed
- Monitor patient conditions and report changes to RN Case Manager or physician
- Educate patients and caregivers on condition management and self-care
- Document visits accurately using EMR systems
- Assist RNs with specialized procedures as needed
- Follow all regulatory and safety compliance standards
Qualifications
- Active LPN license in Ohio (or eligible to obtain)
- Graduate of an accredited Practical Nursing Program
- Minimum 1 year of LPN experience preferred (Home Health preferred, not required)
- Knowledge of Medicare/CMS guidelines is a plus
- Comfortable with electronic charting and independent scheduling
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
Work Environment & Expectations
- Work Setting: Patients’ homes
- Physical Requirements: Ability to lift, bend, walk, and stand throughout shifts
- On-Call Availability: Optional rotations for evenings, weekends, and holidays
- Communication: Prompt notification of schedule changes to Director of Nursing
