Alternative Home Health is seeking a passionate RN to join our growing team. As a RN, you would be able to work independently, be dependable, compassionate and have a desire to help others. The Registered Nurse (RN) will travel to patients' homes and be responsible for administering individualized patient care.

RN CASE MANAGER - HOME HEALTH - RESEPONSIBILITIES:
- Provide skilled services/education for the patient to achieve their optimal level of health while remaining in their home.
- Work with a team and implement a care plan for each patient.
- Communicate effectively with patients, families, physicians, and other healthcare providers.
- Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procedures as appropriate.
- Observes signs and symptoms and reports to the physician reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
- Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Maintains accurate and complete records of observations, treatments and care of patient.
- Takes on-call duty rotation; ranges from every 3rd to 4th weekend.
- Attends staff meetings, patient care conferences and in-services as scheduled.
