Community Support Services Specialist- Case Manager (Dayville, CT)
Description:
Do you want to be the person who gives someone hope? As a Case Manager in our Community Support Program (CSP), you will build meaningful, connections with adults navigating serious and prolonged mental illness. In this role, you will help teach life skills, secure safe housing, access quality medical care, build personal relationships, feel connected to their community and reach their personal goals.

What You Will Do (Day-to-Day):
- Teach Life Skills: Empower clients to manage daily routines, health, and wellness goals.
- Coordinate Care: Link clients to essential mental health, medical, and community resources.
- Build Connections: Form trusted, supportive relationships to guide long-term recovery.
- Advocate daily: Help clients navigate benefits, housing systems, connect to community resources and secure productive activities or work.
What You Bring (Must-Haves)
- A compassionate, hope-driven approach to client care.
- Experience working with mental health, substance use, or community support services.
- An understanding of psychosocial issues affecting vulnerable populations.
Work Location: In-person. This role requires a hands-on presence to deliver the best care possible. You will work directly with clients on-site and in the local community.
Use of agency vehicles as well as use of own car (mileage and reimbursement).
Schedule:
Monday through Friday, 8:30 a.m. – 5:00 p.m. with one evening until 7:00 p.m.
Minimum Qualifications:
Bachelor’s degree in a relevant human services field OR Associate’s degree in a relevant human services field plus two years of relevant experience required.
Hourly Pay Range: $19.00 – $21.50 (depending on experience)
Bilingual Spanish/English Fluency: $19.96 – $22.59 per hour (depending on experience)
