This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Coastal Empire Community Mental Health Center, 1050 Ribaut Road, Beaufort, SC 29902.
Under general supervision of the Clinic Director, you will provide advanced evidence based clinical services to patients in Beaufort County and for CECMHC. Employee will provide patients with serious mental illness clinical services to include assessments, individual, family, and group counseling, and assess for crisis when needed. You will also serve as a liaison to Beaufort County collaborative partners and will ensure linkages for therapeutic needs to promote wellbeing for patients. In addition employee will serve as a Lead Clinician for the Treatment Team.
Responsibilities for the Mental Health Professional II - Adult Services:
Provide advanced, evidenced based treatment to patients served in the clinic and/or community setting. Provide services that include mental health assessments, individual, group, and family therapy as appropriate. Provide crisis intervention, conduct suicide screenings and develop Personal Safety Plans. Ensure patients are seen as per LOC guidelines; address patient barriers to services; ensure crises are staffed; ensure internal & external customer satisfaction. Meet patient care hour standards.
Provide consultation/educational activities and coordinate with families, caregivers, case managers, and other treatment team members. Participate in interdisciplinary meetings and provide input as needed. Coordinate care to ensure consistency across settings.
Act as Lead Clinician for the Beaufort Adult Team and other Treatment Teams; coordinate closely with the Clinic Director and Child, Adolescent and Families (CAF) Director and serves as a liaison.
Complete documentation is according to QI policies. Document services and contracts to patients. Keep billing disallowances to a minimum. Produce outcomes and/or progress summaries that reflect patient improvement; Write objectives that are specific, measurable, achievable, relevant and time-limited. Write objectives and interventions that address patients' SNAPs. Meet collaborative documentation standards.
Adhere to Center, OMH, & State Corporate Compliance policies, procedures, and responsibilities.
Participate in ongoing professional development. Actively work towards licensure/ professional development, and/or maintain license/certifications. Comply with training requirements. Attend supervision. Participates in other meetings as required.
