Responsibilities
- Conduct thorough patient assessments, including medical history review, physiological evaluation, and mental health screening to determine individualized care needs.
- Develop and implement tailored case management plans that encompass inpatient, outpatient, post-acute, and community-based services.
- Coordinate discharge planning by collaborating with healthcare teams, patients, families, and community resources to ensure safe transitions of care.
- Facilitate diagnostic evaluations and behavioral health assessments for diverse populations, including children and individuals with disabilities or substance use disorders.
- Provide patient service through active communication, education on medical terminology, and support for navigating healthcare systems.
Experience
- Proven hospital experience with a strong background in acute care settings such as mental health hospital and group homes.
- Experience working with adults with severe mental health conditions, or those with substance use disorders.
- Familiarity with managed care processes, utilization management, and discharge planning across inpatient and outpatient environments.
- Proficiency in EMR/EHR systems, medical records management, and medical terminology essential for accurate documentation.
- Demonstrated ability to work effectively with diverse cultural backgrounds and coordinate community resources for comprehensive patient support.
- Strong skills in client communication, social work principles, task prioritization, and working collaboratively with multidisciplinary teams.
- Relevant certifications such as Basic Life Support (BLS), CPR, and experience working with individuals with disabilities or behavioral health needs are highly valued.
