What You'll Do
- Serve as the primary clinical case manager for hospice patients throughout their journey.
- Complete comprehensive assessments and develop individualized plans of care.
- Coordinate care with physicians, social workers, chaplains, hospice aides, volunteers, therapists, and the interdisciplinary team.
- Manage symptoms, provide pain management education, and advocate for patient comfort and quality of life.
- Educate and support patients and families through complex healthcare decisions.
- Collaborate with hospitals, skilled nursing facilities, assisted living communities, physicians, and discharge planners to ensure smooth transitions into hospice care.
- Maintain accurate documentation and contribute to exceptional patient outcomes.
What We're Looking For
- Active Minnesota RN license.
- Experience in hospice, oncology, palliative care, home health, or other advanced clinical settings preferred.
- Strong clinical judgment and exceptional communication skills.
- Passion for educating and supporting patients and families.
- Ability to work independently while collaborating with an interdisciplinary team.
- Valid driver's license and willingness to travel throughout the Greater Twin Cities Metro.
