Riverbank Post Acute is a 99-bed Medicare and Medi-Cal certified skilled nursing facility and is one of the newest additions to the American Specialty Management group based in Modesto, California.
We are seeking a dedicated and compassionate Registered Dietitian to join our skilled nursing facility team. Your expertise in nutrition science and your ability to communicate effectively will be key in educating both staff and residents about proper dietary habits and nutritional information. RESPONSIBILITIES:
Conduct comprehensive nutritional assessments for residents.
Develop and implement individualized nutrition care plans in collaboration with interdisciplinary teams.
Monitor the nutritional status of residents and adjust care plans as necessary.
Provide education and support to residents and their families about dietary needs and food choices.
Ensure compliance with federal and state regulations regarding dietary services in skilled nursing facilities.
Conduct in-service training for nursing staff on topics related to nutrition and dietary management.
Participate in quality improvement initiatives related to dietary services.
QUALIFICATION:
Registered Dietitian (RD) credential required.
Experience working in skilled nursing facilities or similar environments is preferred.
Bachelor's degree in Nutrition, Dietetics, or a related field.
State licensure or certification as required by local laws.
Strong knowledge of therapeutic diets and nutritional care plans.
Excellent communication and interpersonal skills to engage with residents and staff.
Ability to work collaboratively in a team-oriented environment.
