- Provide direct resident care, including assessments, monitoring, and documentation
- Administer medications and treatments in accordance with physician orders and facility policies
- Observe and report changes in residents’ conditions to the healthcare team
- Coordinate care with physicians, families, and interdisciplinary staff
- Supervise and support resident caregivers as assigned
- Assist with plans of care and ensure individualized care needs are met
- Maintain accurate medical records and compliance with state regulations
- Promote a safe, clean, and organized environment for residents
Requirements:
- Current, active LPN license in the state
- Graduate of an accredited nursing program
- CPR/BLS certification (or ability to obtain)
- Previous experience in assisted living, long-term care, or geriatrics preferred
- Strong clinical, organizational, and communication skills
- Ability to work collaboratively and lead support staff
- Compassionate, patient-focused approach to care
- Reliable attendance and flexibility with scheduling
