We are currently seeking a Full-Time hospice RN that organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The hospice RN plans and delivers care to patients by assessment, planning, implementation and effectively interacts with patients, families, and the interdisciplinary team members while maintaining standards of professional nursing and clinical competency.
Patient Care Responsibilities:
- Completes an initial comprehensive physical assessment and history of current and previous illnesses.
- Ongoing comprehensive assessment of patient and family to determine hospice plan of care.
- Develop and revise care plans in collaboration with IDG, PCP, patient & families.
- Initially and continually educates patient and families on hospice services & philosophy.
- Establish HHA care plans as well as indirectly and directly supervising the plan of care per regulations.
- Excellent communication with Clinical Director, IDG Team, PCP, community health representatives, patient & family.
- Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
- Knowledge of and availability to perform patient information visits or admissions as needed including explanation of the hospice services.
- Attend and participate in interdisciplinary group meetings.
Qualifications:
- Graduate of an accredited school of nursing
- Valid license as a Registered Nurse in the State of Illinois
- Valid IL driver's license and auto insurance
- Hospice experience preferred but not required
