Remote RPM Alerts Care Coordinator (LPN/LVN)
TimeDoc Health is seeking compassionate and patient-focused LPN to join our RPM Alerts Team and help patients with chronic conditions stay healthier and out of the hospital.

Respond to Remote Patient Monitoring (RPM) alerts and conduct patient outreach

Support patients with chronic conditions through Chronic Care Management (CCM) and RPM services

Educate patients about their health conditions and help them achieve their care goals

Coordinate with providers, office staff, caregivers, and community resources to ensure patients receive the support they need

Document patient interactions, update care plans, and identify barriers to care
This is a phone-based care coordination role, with team members spending the majority of their day speaking with patients and documenting care.

Active Multi-State LPN License required

Minimum 1 year of LPN experience

Bilingual (English/Spanish) preferred

Strong communication, documentation, and critical thinking skills

Care coordination, case management, call center, triage, or geriatric experience is a plus

8–29 hours per week

Some weekdays evenings available

3 out of 4 weekends on average

Flexible 4-hour shift options

Must work at least 2 major holidays per year
