We are seeking a Certified Nursing Assistant (CNA) to join our team of dedicated healthcare professionals at Swan Lake Nursing & Rehabilitation in Patchogue, New York. As a CNA, you will play a vital role in providing high-quality care to our residents, working under the supervision of our registered nurses and licensed practical nurses. Perform selected resident care activities, including assistance with daily living tasks, bathing, grooming, and dressing
Assist with feeding, toileting, and ambulation as needed
Provide vital sign monitoring, including temperature, pulse, and blood pressure
Assist with transportation of residents between areas of the facility
Collect and transport specimens as needed
Respond to call bells and alarms in a timely manner
Ensure personal alarms are in place and functioning according to individual residents' plans of care
Communicate any changes in residents' conditions to the nurse
Record intake, output, BM, vital signs, toileting, range of motion, ambulation, nutrition, ADL care, security device bracelets, showers/bathing, and skin checks as per facility policy
Assist with preparing residents for examinations and treatments
Demonstrate appropriate restraint application and release
Maintain a clean and safe environment for residents and staff
