Primary Function: Assessment and treatment of clients’ needs, situations and support network to determine their goals and apply in a cognitive therapy setting. Serve as clients’ advocate. Design and implement individual therapy sessions consistent with client goals.
Requirements:
· Experience in the areas of cognition, perception, emotion, and neuropsychology.
· CPR and First Aid Certified.
· Completion of specific procedural skills competency
· Master’s degree in social work , psychology, education, counseling or related human services field.
Duties and Responsibilities:

o Assess client needs in accordance with diagnosis and prescription for treatment.
o Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
o Develop and revise rehabilitation treatment goals and objectives with Supervisor.
o Design and implement individual treatment sessions consistent with established goals.
o Monitor client progress, response to treatment, and provide revisions to treatment plan as necessary.
o Actively participate in ensuring that cognitive rehabilitation therapy services are provided based on the client’s plan of care within the identified time frames (therapy projections)
o Maintain all appropriate documentation within a reasonable period and in compliance with regulations.
o Attend and participate in weekly treatment team meetings.
o Work with Clinical coordinator and CRT supervisor to make changes within the care plan of clients.
o Work with Clinical coordinator to schedule/plan meetings with Family and Universal Team members as needed.
o Participates in team meetings for the purpose of developing service plans to address the needs of clients and their families.
o Continually assesses the adequacy of client service plans and when necessary, revise to achieve goals and objectives.
o Identifies appropriate community resources for clients, refers and links them to those resources and advocates on behalf of clients for resource service delivery.
o Attends staff meetings, individual and group conferences and training programs designed to develop performance and case management skills and that will continuously familiarize staff with current methods and techniques in the field of social services.
o Performs related work as required.
o Provide one-on-one or small-group behavioral interventions as outlined in treatment plans.
o Support individuals in developing communication, social, self-help, and daily living skills.
o Implement behavior support strategies to increase desired behaviors and reduce challenging behaviors
o Follow behavior intervention plans developed by a supervising clinician (e.g., BCBA, behavior specialist, psychologist).
o Use approved teaching and reinforcement techniques consistently and accurately.
o Promote positive behavioral outcomes while maintaining client dignity and safety.

o Compassionate and able to relate to different clients with various needs
o Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records
o Critical thinking and problem solving to determine the best care plan for each client after assessing clients
o Participation in special committees and task groups when requested.
o Flexibility to change care plans if they are not getting the best results
o Organization to manage several different cases at once.
o Computer literacy to maintain and manage case records.
Physical Demands:
· On a daily basis, frequent physical and other interaction with brain injured individuals.
· On a daily basis, approximately 40% of time spent standing, 40% walking, and 20% sitting.
· Occasionally required to lift, carry and push objects and individuals with the approximate weight of 50-75 pounds.
· Occasionally required to stoop, kneel, crouch, crawl, and reach.
· Must be able to speak, hear and see. Depth perception is required.
Working Conditions
· Involves regular interaction with a wide variety of unpredictable and often inappropriate conduct and comments by brain-injured individuals.
