Licensed Practical Nurse (LPN) – PRN Skilled Home Health
Position Summary
We are seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to provide high-quality home health nursing services to patients in their homes on an as-needed (PRN) basis. The LPN will work under the supervision of a Registered Nurse (RN) and in collaboration with the interdisciplinary healthcare team to deliver skilled nursing care in accordance with physician orders, agency policies, and applicable federal and state regulations.
Essential Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with the individualized plan of care.
- Perform nursing assessments and report changes in patient condition to the supervising Registered Nurse and physician as appropriate.
- Administer prescribed medications and treatments safely and accurately.
- Perform wound care, dressing changes, catheter care, ostomy care, and other skilled nursing procedures within the LPN scope of practice.
- Monitor and document vital signs, pain levels, blood glucose, and other clinical observations.
- Educate patients and caregivers on disease management, medications, treatments, and home safety.
- Maintain accurate, timely, and complete clinical documentation in the electronic medical record.
- Communicate effectively with physicians, Registered Nurses, therapists, case managers, and other members of the healthcare team.
- Observe infection prevention and control practices and maintain patient confidentiality in compliance with HIPAA.
- Participate in case conferences, quality improvement activities, and ongoing education as requested.
- Respond to PRN scheduling requests based on agency needs and availability.
Qualifications
- Current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
- Current CPR/BLS certification.
- Minimum of one (1) year of nursing experience preferred; home health experience is strongly preferred.
- Knowledge of home health regulations and standards of practice preferred.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to pass required background screening and drug testing, if applicable.

Physical Requirements
- Ability to lift, transfer, and assist patients as needed (up to 50 pounds with or without reasonable accommodation).
- Ability to stand, walk, bend, kneel, and reach throughout the workday.
- Ability to travel between patient homes.
Schedule
- PRN (as needed)

- Weekend, evening, and holiday availability may be required.
