Position Summary
We are seeking a compassionate, experienced, and patient-focused Mental Health Counselor to provide behavioral health counseling and clinical support to patients.
The role includes mental health counseling, Patient education including medication management education and support, diabetes education, and smoking-cessation counseling. The counselor will collaborate with physicians, psychiatrists, and other healthcare professionals to support comprehensive patient care.
All services must be performed within the individual's education, training, credentials, applicable state laws, and organizational policies.
Key Responsibilities1. Mental Health Counseling
· Provide individual mental health counseling sessions to patients.
· Assess patients' emotional, behavioral, and psychosocial needs.
· Support patients experiencing anxiety, depression, stress, mood-related concerns, and other mental health conditions within scope.
· Develop patient-centered goals and counseling plans.
· Use appropriate evidence-based counseling techniques.
· Monitor patient progress and document outcomes.
· Provide supportive counseling and coping strategies.
· Recognize and appropriately escalate safety concerns.
2. Medication Management Support
· Review and document medication information as part of the patient's care process.
· Provide education regarding prescribed medications, adherence, proper use, potential side effects, and precautions.
· Monitor and document patient-reported medication concerns.
· Communicate medication-related concerns to the prescribing provider.
· Support medication adherence and follow-up.
· Perform medication-related activities only within the individual's applicable scope of practice and organizational protocols.
3. Diabetes Education
· Provide diabetes education and self-management support.
· Educate patients on blood glucose monitoring and appropriate self-care.
· Reinforce education regarding medication adherence, nutrition, physical activity, and lifestyle modifications.
· Help patients understand and follow individualized diabetes care plans.
· Encourage healthy lifestyle changes and disease-management practices.
· Identify concerning symptoms or changes in the patient's condition and notify the appropriate provider.
· Document diabetes education and patient participation.
4. Smoking-Cessation Counseling
· Conduct individual smoking-cessation counseling sessions.
· Educate patients regarding the health risks associated with tobacco and nicotine use.
· Assess readiness to quit and establish individualized cessation goals.
· Provide behavioral counseling, motivation, and ongoing support.
· Assist patients in identifying triggers and developing strategies to reduce or stop tobacco use.
· Monitor patient progress and provide follow-up counseling.
5. Patient Assessment & Care Coordination
· Conduct appropriate behavioral health and psychosocial assessments.
· Identify patient needs and recommend appropriate clinical follow-up.
· Coordinate care with physicians, psychiatrists, nurse practitioners, pharmacists, and other healthcare professionals.
· Refer patients to appropriate clinical or community resources when necessary.
· Participate in multidisciplinary care coordination and case discussions.
· Maintain appropriate patient follow-up.
6. Documentation & Compliance
· Complete accurate and timely clinical documentation for all patient encounters.
· Maintain counseling notes, assessments, treatment plans, education records, and follow-up documentation.
· Ensure documentation supports the services provided and applicable billing requirements.
· Maintain patient confidentiality and comply with HIPAA and applicable privacy regulations.
· Follow organizational policies, clinical protocols, SOPs, and quality standards.
· Maintain all required professional licenses and certifications.
Required Qualifications
· Master's degree or higher in Clinical Mental Health Counseling, Counseling, Psychology, Mental Health Counselor (LPC/LMHC) or another related behavioral health discipline, as applicable to the position and state requirements.
· Current and appropriate professional license or credential required for the services being provided.
· Experience in behavioral health, psychiatry, counseling, or mental health services preferred.
· Strong understanding of behavioral health principles and patient-centered care.
· Excellent verbal and written communication skills.
· Strong patient counseling and education skills.
· Ability to maintain accurate and timely clinical documentation.

· Outpatient behavioral health or mental health services.
· Mental health counseling and psychotherapy.

· Telehealth behavioral health services.
· Electronic Health Record (EHR) experience.

Scope of Practice
All clinical services must be performed within the individual's education, training, credentials, applicable state laws, and organizational policies.
Medication prescribing or independent medication management may only be performed by a clinician who holds the appropriate legal authority and credentials to prescribe or manage medications independently.
The organization may modify responsibilities based on clinical needs, regulatory requirements, and organizational policies.
