Wound Care Nurse (RN/LPN) – Full-Time, Monday–Friday
Position Summary
We are seeking a compassionate and clinically skilled Wound Care Nurse (RN/LPN) to oversee and coordinate wound care services for residents in our skilled nursing and rehabilitation community. The Wound Care Nurse is responsible for assessing, treating, monitoring, and documenting wounds while collaborating with the interdisciplinary team to promote optimal healing and positive clinical outcomes.

- Participation in an on-call rotation as assigned
Essential Responsibilities
- Conduct comprehensive skin and wound assessments.
- Develop, implement, and evaluate individualized wound treatment plans.
- Complete weekly wound rounds and maintain accurate measurements, photographs, and documentation.
- Monitor healing progress and communicate changes in condition to physicians and the interdisciplinary team.
- Provide recommendations for pressure injury prevention and skin integrity management.
- Educate residents, families, and staff regarding wound prevention and treatment protocols.
- Ensure compliance with facility policies, state and federal regulations, and best-practice standards.
- Participate in quality improvement initiatives related to wound care and skin management.
- Serve as a clinical resource and mentor for nursing staff regarding wound care practices.
Qualifications
- Current Florida RN or LPN license in good standing.
- Experience in long-term care, skilled nursing, rehabilitation, or wound care preferred.
- Wound Care Certification preferred or willingness to obtain.
- Strong assessment, organizational, and communication skills.
- Ability to work collaboratively with physicians, nursing staff, therapy, and dietary services.
