Comply with state practice act and/or American Speech-Language-Hearing Association (ASHA) guidelines.
Develop and implement treatment plans that focus on functional and measurable outcomes with physician order. Monitor residents’ responses to treatment and alter the treatment plan as appropriate with physician approval.
Evaluate, diagnose, and treat speech, language, voice, cognitive-communication, and swallowing disorders.
Participate in continuous quality improvement activities.
Identify and advise Rehabilitation Manager (RM) of any Center, staff, or operations issues that need to be addressed.
Communicate resident/staff issues affecting assigned caseload.
Develop and implement Speech Therapy and Dysphagia services in conjunction with the RM and Center staff.
Participate in discharge planning.
Demonstrate supervisory activities in patient charts and service logs as applicable.
Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehabilitation services.
Provide speech, language, cognitive, and swallowing screenings and evaluations to determine residents’ functional level before initiation of treatment.
Maintain productivity at level determined by the RM.
Supervise Speech-Language Pathology Assistants and aides if applicable.
Self-monitor productivity level daily.
Participate in resident/family meetings as well as Center meetings when appropriate.
Educate residents, families, and caregivers regarding treatment goals, progress, and home programs.
Follow administrative policies and procedures related to emergency procedures, infection control, and safety.
Maintain consistent and predictable hours of service in buildings as assigned.
Provide clear, concise documentation in medical records and on appropriate forms as it relates to reimbursement guidelines and Center policy and procedure.
Document and sign each treatment encounter in medical records.
Complete progress notes per Center policy.
Participate in Quality Assurance Review and denials process as requested by RM.
