Job Opportunity: Seeking Experienced Field Registered Nurses
Our client is seeking dedicated and skilled Field Registered Nurses to join their dynamic team in upstate New York. This hybrid role combines both in-person and telehealth visits, offering flexibility and rewarding opportunities to provide exceptional care.

- Operational Hours: Monday - Friday 7:30AM - 8:00PM. Saturdays and Sundays 10:00AM - 6:30PM
Key Responsibilities:
- Conduct initial in-home evaluations of patients using the UAS-NY assessment tool to assess health status, strengths, and care needs.
- Develop and maintain positive relationships with consumers, caregivers, and facility staff.
- Stay current with UAS-NY assessment processes and updates.
- Review and analyze medical documentation and health referral forms.
- Enter and transmit evaluation data through the electronic evaluation system.
- Document and address any concerns or discrepancies identified during evaluations.
- Facilitate communication between the Call Center and eligible consumers for managed care enrollment.
Qualifications:
- Active New York State RN License: Must be unencumbered.
- Experience: Minimum of two years in a field-based visiting nurse role.
- Preferred: Familiarity with the UAS assessment tool (training provided).
- Valid Identification: MUST have a NYS driver's license or NON-DRIVER ID
- Reliable Internet: Access to high-speed service.
