Job Overview
We are seeking a dedicated and skilled Occupational Therapist (OT) to provide comprehensive home health services to diverse patient populations. The ideal candidate will possess a strong background in patient care, with expertise in assisting with activities of daily living (ADLs), discharge planning, and working with individuals across various age groups and conditions. As an OT in our home health agency, you will play a vital role in developing personalized care plans, promoting independence, and improving quality of life for patients recovering from illness or injury, managing disabilities, or undergoing rehabilitation.
Responsibilities
- Conduct thorough assessments of patients’ functional abilities, including mobility, self-care, and cognitive skills.
- Develop and implement individualized treatment plans tailored to each patient’s needs, goals, and medical conditions.
- Assist patients with activities of daily living (ADLs), such as dressing, grooming, and feeding to promote independence.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings.
- Work with individuals with autism, disabilities, geriatrics, pediatrics, and those requiring post-acute or outpatient care.
- Provide education to patients and caregivers on therapeutic exercises, adaptive techniques, and safety measures.
- Maintain accurate documentation of patient progress, treatment interventions, and care plans in compliance with regulatory standards.
- Engage in case management activities to monitor ongoing needs and adjust treatment strategies accordingly.
- Utilize knowledge of physiology, anatomy, medical terminology, and speech therapy principles to inform effective interventions.
- Participate in skilled nursing facility visits or inpatient settings when necessary to support comprehensive patient care.
Experience
- Proven experience as an Occupational Therapist within home health settings or outpatient clinics.
- Demonstrated ability to work effectively with individuals with disabilities across various age groups including pediatrics and geriatrics.
- Experience assisting with ADLs and implementing discharge planning strategies for diverse patient populations.
- Background working in acute care environments such as ICU or inpatient units is highly desirable.
- Familiarity with early intervention programs and school-based occupational therapy is advantageous.
- Knowledge of medical terminology, anatomy, physiology, and therapeutic techniques relevant to occupational therapy practice.
- Prior experience in skilled nursing facilities or post-acute care environments is preferred.

Pay: $32.27 - $38.86 per hour
