Position Overview
The Resource Navigation Coordinator provides day-to-day operational leadership for Latched Support's Resource Navigation Program, ensuring families receive timely, compassionate, and high-quality support while helping the program operate efficiently across community and hospital settings.
This position oversees the daily coordination of Resource Navigation Associates and Resource Navigation services, supports the hospital resource navigation initiative, maintains a reduced caseload of families, and works closely with the Program Manager to improve workflows, strengthen community partnerships, and monitor program quality and outcomes.
The Resource Navigation Coordinator serves as the primary operational leader for the Resource Navigation team and plays an important role in staff development, program growth, and continuous quality improvement. This position is ideal for an experienced professional who enjoys mentoring others, improving systems, building relationships, and helping families access the resources they need to thrive.

Key ResponsibilitiesProgram Operations & Coordination
- Coordinate the daily operations of the Resource Navigation Program.
- Oversee scheduling and workflow for resource navigation appointments.
- Monitor caseload distribution and ensure timely follow-up with families.
- Support implementation of new workflows, policies, and procedures.
- Identify opportunities to improve efficiency and client experience.
- Ensure compliance with organizational policies, grant requirements, and documentation standards.
- Assist the Program Manager with program planning and operational priorities.
Team Leadership & Staff Development
- Provide day-to-day leadership, guidance, and support to Resource Navigation Associates.
- Assist with onboarding and training new staff.
- Monitor documentation for quality, accuracy, and timeliness.
- Coach staff on best practices for client engagement and resource navigation.
- Assist with scheduling, workload balancing, and operational problem-solving.
- Serve as the first point of contact for daily operational questions and support.
Resource Navigation & Family Support
- Maintain a reduced caseload of families requiring resource navigation services.
- Conduct comprehensive resource assessments.
- Connect families to community resources, healthcare services, educational programs, housing assistance, food assistance, financial resources, and other supportive services.
- Educate families on available Latched Support programs and assist with enrollment.
- Follow up with families to help ensure successful connection to recommended services.
- Document services provided according to organizational guidelines.
Hospital Resource Navigation Initiative
- Coordinate day-to-day operations of Latched Support's hospital-based Resource Navigation services.
- Support scheduling and coverage for hospital resource navigation staff.
- Monitor hospital referral processes and family follow-up.
- Collaborate with hospital partners to ensure seamless coordination of services.
- Identify opportunities to improve patient engagement and referral outcomes.
- Assist with expansion of hospital resource navigation initiatives as organizational growth allows.
Community Partnerships & Outreach
- Maintain positive working relationships with community organizations, healthcare providers, schools, and referral partners.
- Represent Latched Support at outreach events, resource fairs, and partner meetings.
- Help identify new community resources and partnership opportunities.
- Promote Latched Support programs throughout the community.
Data, Compliance & Quality Improvement
- Monitor program performance metrics and operational outcomes.
- Review referral completion rates and identify service gaps.
- Assist with grant reporting and data collection.
- Ensure documentation meets organizational and funding requirements.
- Support continuous quality improvement initiatives.
- Assist with preparing reports for leadership when requested.
Additional Responsibilities

- Participate in leadership meetings and program planning.

Preferred Experience
- Community Health Worker (CHW) Certification strongly preferred.
- Minimum three years of experience in community health, resource navigation, case management, social services, maternal-child health, or a related field.
- Previous leadership, supervisory, or team lead experience preferred.
- Experience working with pregnant or parenting families strongly preferred.
- Experience building community partnerships.
- Bilingual (English/Spanish) strongly preferred.
Additional Requirements
Background Check
Must successfully pass DFPS, DPS, and NSOPW background screenings.
Work Hours & Travel
May require occasional evenings and weekends to support community events, hospital initiatives, or organizational needs. Reliable transportation required for travel throughout the San Antonio area.
Physical Requirements
Must be able to lift up to 40 pounds, stand for extended periods, and travel to community and hospital locations.
Work Environment & Safety
Work may take place in offices, hospitals, community centers, healthcare facilities, schools, and community events. Employees must comply with all organizational safety policies, HIPAA regulations, infection prevention protocols, and confidentiality requirements.
