Practice Beyond the Traditional Clinic.
You'll provide comprehensive primary care throughout assisted living, memory care, skilled nursing, and senior housing communities across the Brainerd Lakes region.
Rather than spending your day inside the same clinic, you'll travel between communities, managing acute illnesses, chronic diseases, preventative care, and care transitions while collaborating closely with physicians, nurses, therapists, caregivers, and facility leadership.

What You'll Be Doing
- Conduct comprehensive patient assessments and routine provider visits
- Diagnose and manage acute and chronic medical conditions
- Develop individualized treatment plans
- Identify changes in condition and intervene proactively
- Coordinate care following hospitalization and rehabilitation
- Collaborate with physicians and interdisciplinary healthcare teams
- Educate patients, families, and caregivers on treatment plans and disease management
- Participate in specialty programs and quality improvement initiatives
- Help reduce unnecessary hospitalizations and improve long-term patient outcomes

What We're Looking For
Required Qualifications
- Graduate of an accredited Physician Assistant program
- Active Minnesota PA-C license in good standing
- Strong assessment, diagnostic, and clinical decision-making skills
- Ability to practice independently while collaborating with physicians and interdisciplinary teams
- Passion for caring for older adults and medically complex populations
- Valid driver's license and reliable transportation
