What You'll Do:
Coordinate the intake of patients referrals and ensure timely admission to home health services
Communicate effectively with agency staff and external partners
Manage multiple tasks and projects with professionalism and accuracy

What You'll Bring:
High School Diploma; some college preferred
1+ years of experience in a similar role, Medicare Home Health experience preferred
Organized team player with a positive attitude and strong communication skills
Ability to multitask and stay detail-oriented in a fast paced environment
Comfortable using MS Office, Excel, EMR systems, and company websites
